We offer processing of NEW passports by appointment and we take passport photos at any time, just drop in.


Appointments are available Tuesdays, Wednesdays and Thursdays from 2-5 p.m. and 6-8 p.m.; and Saturdays from 10 a.m. - 1 p.m. at our 2800 Tremont Road location.

If you would like to renew your passport, please follow these instructions. We do not process renewals.

Due to high demand we ask that you make only one appointment per applicant per week. In case of multiple appointments for the same person, we reserve the right to keep only the earliest appointment and cancel the others for the week.

I am Applying for a New Passport: What Should I Bring?

  • Proof of U.S. citizenship
  • Current Ohio Driver’s License, Military ID or Government ID
  • Additional documentation required for applicants under age 16
  • Completed application Form DS-11 (Download to fill out by hand or Complete online and print)
  • One recent (not older than 6 months), color 2"×2" photo of applicant. The Library can take a photo if preferred (for applicants 3 years of age or older).

The Passport Photo

Update: As of November 1, 2016 applicants must remove their eye glasses when taking a photo for a U.S. passport. The only exception are rare cases of medical necessity and those applicants will be required to have a signed statement from a medical professional.

How much does a new U.S. passport cost and how do I pay?

  • Passport fees
    • Passport Book: $110/adult, $80/minor — paid by check or money order only and payable to U.S. Department of State (no starter or counter checks accepted)
    • Passport Card: $30/adult, $15/minor — paid by check or money order only and payable to U.S. Department of State (no starter or counter checks accepted)
    • Execution fee: $25 — paid by cash, check or money order only and payable to the Upper Arlington Public Library
  • Photo fee (if applicable) — $10* per photo
