I Need a New Passport

Please schedule an appointment with our passport agents if you are applying with a DS-11 form.

You are eligible if:

  • You are applying for your first U.S. passport
  • You are under age 16
  • Your previous U.S. passport was issued when you were under age 16
  • Your previous U.S. passport was lost, stolen, or damaged
  • Your previous U.S. passport was issued more than 15 years ago

All parents and legal guardian(s) must attend the appointment for applicants under 16. See travel.state.gov for exceptions.

The Library can take a passport photo onsite for $15 for applicants with an appointment to execute a new passport (ages 3 or older).

Two (2) separate payments are required per application:

  1. One check or money order addressed to the U.S. Department of State
  2. Payment to the UAPL by cash, check or credit card

Incomplete applications or missing payment will result in a cancelled appointment.

Appointments are available Tuesdays, Wednesdays, and Thursdays at our 2800 Tremont Road location.

We require one appointment per passport application. For groups/families with multiple individual applications, please make an appointment for each applicant.

I Need To Renew My Passport

The Library cannot process renewal applications if you are applying with the DS-82 form. Mail your renewal application directly to the State Department or apply online at travel.state.gov.

Applicants under 16 must always re-apply with a DS-11 form, please see the "I need a New Passport" section above to schedule an appointment.

The Library provides physical passport photos for ages 3+ for a fee. We cannot assist with digital photos or online applications.

I Need a Passport Photo Only

Walk-in photo services are available at the Main Library during our open hours up to 30 minutes prior to closing. No appointment is needed.
The Library cannot assist with digital photos at this time.

Most drug stores and photo centers also offer walk-in passport photo services.
