Library Card Application

Thank you for your interest in joining Upper Arlington Public Libraries! All residents of Ohio are eligible to apply for a free library card. To receive your card, please accept the terms of our Borrower's Agreement, complete the application form and bring your photo ID with proof of current address to the help desk in any of our library locations. Borrower's Agreement

I will:

  • Adhere to library policies.
  • Be financially responsible for the loss or damage of all materials borrowed on this card.
  • Report a lost/stolen card immediately; I am solely responsible for items lost/stolen by someone using my card if I haven't reported it missing.
  • Keep my library account up to date with correct address, phone and e-mail address.

I understand:
Should my account be blocked due to long overdue materials, fines, or fees, I will be contacted by mail or e-mail. Unresolved account delinquencies may be turned over to Unique Management Services for collection.

Children under the age of 18 must be accompanied by a parent or legal guardian in order to receive their library card. The Library will hold parents/legal guardians responsible for the content of materials borrowed and any fees or fines incurred with this card. The library system's collection serves a wide diversity of people and contains materials which some parents/guardians may believe are inappropriate for their children. It is the responsibility of the parents/guardians to monitor the material their child selects and checks out on this card. Individuals who reach the age of 18 during the life of the library card are asked to update their records to an adult card at the Help Desk.
